New Student Enrollment Forms
Please print and complete the forms listed below.  These forms are required when enrolling a student in the Sheridan School District.
If you are going to paritcipate on a school sports team the form below must be completed by a physician.
According to state law, Sheridan School District cannot allow your child to attendschool unless we receive evidence that the minimum immunization requirements have been met.
